Required Forms for New Students

⇔ STUDENT/FAMILY INFORMATION FORM: This form will be used by the Registrar and Business Office to mail information to the appropriate locations, both during term and during breaks.

⇔ MEDICAL HISTORY FORMS (Student Athletes will be receiving different medical history forms):

Student/Non-Athlete Form - All students must provide proof of immunizations and have the form signed by a licensed healthcare provider. It is a Vermont State law that verification of immunizations by a healthcare provider be submitted before entering any college. If you do not provide this information in a timely manner, later in the semester there will be a "hold" placed on future registration and activities. Immunizations can be obtained in Health Services. However, the bill for these has to be paid in full at the time of immunization. it is best to obtain these immunizations prior to arrival to campus.
If you are a Nursing or Radiologic Sciences student, you must fill out the additional section and complete a physical.

⇔ RESIDENCE LIFE PACKET: If you will be living on campus and/or are interested in a meal plan or having Mountain Money, please return this form as soon as possible but no later than January 6. Note that rooms are generally assigned on a first-come, first-served basis.

⇔ FAMILY RIGHTS AND PRIVACY ACT (FERPA) OF 1974: Included at the end of the FERPA letter is a section to sign if you DO NOT wish to have directory information to be disclosed to third parties.

⇔ FERPA WAIVER FORM: Sign and return if you DO want academic or behavioral information disclosed to your parent(s)/guardian(s). If you have any questions, you may contact James Frederick, Registrar, at 802-447-6324 or registrar@svc.edu.
