Learning Differences Support Program Application

Program Application and Eligibility
To be eligible to participate in the Learning Differences Support Program, students must submit a written statement from a qualified psychologist or doctor verifying the disability and send recent documentation specifying the nature of the specific learning disability, ADHD, physical, or other impairment. This information helps the disabilities specialists plan appropriate support for each individual. An interview with the program staff is strongly recommended.
The following documentation is required for learning disabilities:
- A recent WAIS-R (within 2-3 years) with subtest scaled scores.
Participants in the program should have at least average WAIS scores in the areas of reasoning and conceptual ability and a profile which suggests that they have the ability to succeed in college despite language-based disabilities and/or other deficit areas.
- A psycho-educational evaluation specifying the nature of the learning disability.
- Individually administered achievement tests in reading skills/comprehension, math, and spelling with grade equivalents and percentiles. Documented attainment of at least a 9th-grade level of reading
comprehension is highly recommended.
- IEP (Individualized Educational Plan) or 504 Plan if available.

For ADHD, vision and hearing impairments, or other disabilities, recent documentation and diagnostic evaluations (within two years) must be provided by an appropriate certifying professional.
Frequently Asked Questions
When do I request accommodations and support?
- All students will receive a Request for Accommodation/Disability Disclosure form upon acceptance to the College.
- To request accommodations and support, the student must complete and return this form to the Learning Differences Support Office as soon as possible.
- After receiving the completed request form, the Learning Differences Support Office will provide the student with information about the program and documentation required for eligibility.
Where do I send the required documentation?
- All documentation should be sent to the Learning Differences Support Coordinator as soon as possible.
- Documentation and evaluation information remain confidential.
